How to authenticate Single Status Affidavit for use in China?

Any individuals who are planning to register for marriage in China are required to apply for an Affidavit of Single Status authentication, or sometimes called Single Status Certificate/Statement/Affidavit.

As local marriage offices in China may have special requirements or preferences for filling out your affidavit, it is best to check with your respective office for details. We highly recommend to send a draft to the office for approval before you proceed with the authentication. CEV is not liable for any errors or discrepancies in the contents or completion of your form. An authenticated Affidavit of Single Status is usually valid for 6 months starting from the signature date.


How to complete an Affidavit of Single Status for use in China?

Please download the pre-formatted Affidavit of Single Status Form and fill it out.

After the form has been completed, you must authenticate the documents in the following order:

1.) Notarization by local Notary Public

2.) Authentication by Secretary of State

2a.) Authentication by US Department of State for the following states: Delaware, Idaho, Kentucky, Maryland, Montana, Nebraska, North Carolina, North Dakota, South Carolina, South Dakota, Tennessee, Utah, Virginia, West Virginia, Wyoming and Washington DC.

3.) Authentication by Chinese Consulate or Chinese Embassy
